The most common hormonal disruption in endurance athletes is relative energy deficiency in sport, which suppresses the hypothalamic-pituitary axis and reduces production of sex hormones, thyroid hormones, and growth factors. Male athletes may experience reduced testosterone. Female athletes may experience menstrual irregularity or amenorrhea. Research published in the British Journal of Sports Medicine identified low energy availability as the primary driver of hormonal disruption in athletes. Overtraining syndrome also manifests with hormonal markers including elevated cortisol and reduced testosterone. Supplements cannot fix hormone imbalances caused by inadequate energy availability. The primary treatment is increasing caloric intake to match energy expenditure. Research published in the Journal of the International Society of Sports Nutrition found that ashwagandha supplementation at 600 milligrams per day for eight weeks increased testosterone levels and improved muscle strength and recovery in healthy men engaged in resistance training. A separate study in the American Journal of Men's Health found testosterone-supporting effects in older, overweight men, suggesting broader applicability beyond trained athletes. The mechanism appears to involve cortisol reduction, which removes a hormonal brake on testosterone production, combined with direct effects on the hypothalamic-pituitary-gonadal axis. The testosterone increase is within the normal physiological range and does not constitute doping.
Vitamin D receptors are present in the testes, ovaries, and adrenal glands, and vitamin D deficiency is associated with lower testosterone levels in men and hormonal irregularity in women. A study published in Hormone and Metabolic Research found that men who supplemented with vitamin D to correct deficiency showed significant increases in total and free testosterone over 12 months. The relationship is corrective rather than supra-physiological: raising vitamin D from deficient to adequate levels restores normal hormone production, but supplementing above adequate levels does not further increase testosterone. Blood testing for vitamin D status guides appropriate supplementation.
DHEA is a precursor hormone that the body converts to testosterone and estrogen. While available as an over-the-counter supplement in some countries, DHEA is on the World Anti-Doping Agency prohibited list and is banned in competition. Athletes subject to drug testing must avoid DHEA supplements entirely. Other hormone precursors marketed for testosterone support, including tribulus terrestris and D-aspartic acid, have shown minimal efficacy in controlled trials. Blood testing through a physician or sports medicine specialist is the definitive diagnostic method. Key markers include total and free testosterone, estradiol, cortisol, thyroid panel including TSH and free T3 and T4, and vitamin D. For female athletes, reproductive hormone panel and menstrual history provide additional diagnostic information. Symptoms including persistent fatigue, decreased performance despite adequate training and recovery, loss of libido, mood changes, and menstrual irregularity warrant evaluation. Regular testing during heavy training periods provides longitudinal data that identifies trends before they become clinically significant problems.
Zinc, vitamin D, magnesium, and ashwagandha are not on the WADA prohibited list and are safe for drug-tested athletes. DHEA, prohormones, and any compound marketed as a testosterone booster that contains androgenic precursors are prohibited. The contamination risk with hormone-support supplements is elevated because they are manufactured in the same category as products that do contain banned substances. Third-party certification from Informed Sport or NSF Certified for Sport is particularly important for this supplement category. Athletes should verify certification for the specific product and batch before use.
